Who is Eligible for Hospice?
Hospice care is reserved for individuals with a prognosis of 6 months or less. This type of palliative care is available to persons with a variety of diseases, including but not limited to:
- Heart Disease
- Kidney Disease
- Pulmonary Disease
- Alzheimer’s and other Dementia’s
- Neurological Diseases
- Cancer
- AIDS
- And any other life-limiting illness.
Patients may be eligible for hospice care if any or all of the following are present:
- Progressive decline in status despite curative treatments
- Frequent hospitalizations
- Repeat or multiple infections
- Increased or uncontrolled pain
- Progressive/profound weakness and fatigue
- Shortness of breath with or without oxygen
- Dependency-decrease in Activity of Daily Living (ADL’s)
- Alterations in mental status
- Unintentional Weight loss
