Who is Eligible for Hospice?

Hospice care is reserved for individuals with a prognosis of 6 months or less. This type of palliative care is available to persons with a variety of diseases, including but not limited to:

  • Heart Disease
  • Kidney Disease
  • Pulmonary Disease
  • Alzheimer’s and other Dementia’s
  • Neurological Diseases
  • Cancer
  • AIDS
  • And any other life-limiting illness.

Patients may be eligible for hospice care if any or all of the following are present:

  • Progressive decline in status despite curative treatments
  • Frequent hospitalizations
  • Repeat or multiple infections
  • Increased or uncontrolled pain
  • Progressive/profound weakness and fatigue
  • Shortness of breath with or without oxygen
  • Dependency-decrease in Activity of Daily Living (ADL’s)
  • Alterations in mental status
  • Unintentional Weight loss
